GVS Plan Options
Fee-For-Service Plan
GVS offers several fee-for-service plans that completely eliminate participants' out-of-pocket expenses while giving them maximum value for your plan's vision dollar. These plans cover all eligible participants (members and/or families) for a comprehensive eye examination, frame, lenses, or contact lenses.
The frequency of service and benefit coverage levels are customized to meet your organization's needs.
There are no administrative costs or co-payments.
We offer two types of fee-for-service plans:
- All Inclusive Price Plan: For one paid-in-full pre-established price, participants will receive an eye exam, frame, lenses, or contact lenses.
- Itemized Price Plan: Based on pre-established, discounted individual prices, participants are eligible for an eye exam, frame, lenses, or contact lenses.
On a monthly basis, your Fund or Third Party Administrator will only be billed for those participants who have utilized this service during the billing period.
Your plan can also be further customized to include many optional eyewear products that can be available to your participants, at prices that reflect considerable savings when compared to Average Retail Prices (ARP).

Fixed Payment Plan
Fixed payment, as its name implies, is a fixed monthly payment per member per month (PMPM) in your organization. "Per Member Per Month" is the healthcare standard payment method and payment period.
For a pre-established fixed monthly fee, all participants (members and/or their families) are entitled to an eye exam, frame, lenses or contact lenses.

Customized Fixed Payment Plans
Your customized Fixed Payment Plan can encompass many enhancements, including safety, computer and sports eyewear, Loss Protection Programs, and LASIK Surgery at reduced rates. Plans are customized for Members only, Members and Families and/or Retirees.
GVS Administers Your Fixed Payment Plan
GVS is your plan's administrator and electronically maintains eligibility, patient history, adjudicates claims and provides you with customized management reports.
Your plan can also be further customized to include many optional eyewear products that can be available to your participants, at prices that reflect considerable savings when compared to Average Retail Prices (ARP).

Payroll Deduction Plan
The Payroll Deduction Plan is a voluntary program by which your payroll department deducts a pre-determined weekly, bi-weekly or monthly payment from your participating members' paycheck. Each member has the option to enroll singularly or with family coverage. Participants are eligible to sign onto the program once every 12 months. There is a one-time initial enrollment fee of $20.00. When the member elects to extend the benefit for the following year(s), there will be no additional fee.
Plan Highlights
- On a yearly basis, Payroll Deduction Plan participants will receive an Eye Exam,
Frame, Lenses or Contact Lenses
- The participant benefit level is dependent upon the payroll deduction
contribution
- All monies being deducted can be pre-tax, giving the participant a definite
tax advantage

needs

One Price Discount Plan
This non-insured, exclusive vision care program gives your members and their families an extremely affordable complete Eyecare and Eyewear Benefits Package that can be used on an UNLIMITED BASIS. One price covers Eye Exam, Frame and Lenses.
This one price discount plan is ideal for non-covered members and dependents or for those participants who have already used their benefits. The plan provides vision benefits not presently offered or to strengthen an already existing benefit package.
Plan members, dependents and extended family members are entitled to the following basic vision benefits on an unlimited basis:
- Professional Eye Examination* Along with determining your prescription, our comprehensive eye exam, performed by a Doctor of Optometry, will also aid in detecting High Blood Pressure, Cataracts, Diabetes, Glaucoma, Retinal Disorders, and
Corneal Disorders.
- Huge selection of various frames** Any style, color and size up to a retail value that is determined upon plan selected.
- All First Quality Single Vision, Conventional Bifocal, Safety & Oversize lenses.
- Cosmetic & Sunglass Tints
- Additionally, Contact Lenses and Eye Exams are Available
One Price Discount Plans can be customized to include many optical eyewear products at prices that reflect considerable savings when compared to average retail prices (ARP).
There is a small administrative fee for managing this plan.

*Due to state optometry laws eye exam is included only in NY. Outside NY there is a $20 exam fee.
