GVS Plan Options
Every plan member that you service will be covered under one of the 4 following plans:
- Fee-For-Service Plan
- Fixed Payment Plan
- Payroll Deduction Plan
- One Price Discount Plan
Fee-For-Service Plan
GVS offers several fee-for-service plans that completely eliminate participants' out-of-pocket expenses while giving them maximum value for your plan’s vision dollar. These plans cover all eligible participants (members and/or families) for a comprehensive eye examination, frame, lenses, or contact lenses.

Fixed Payment Plan
Fixed payment, as its name implies, is a fixed monthly payment per member per month (PMPM) in your organization. "Per Member Per Month" is the healthcare standard payment method and payment period.
For a pre-established fixed monthly fee, all participants (members and/or their families) are entitled to an eye exam, frame, lenses or contact lenses.

Payroll Deduction Plan
The Payroll Deduction Plan is a voluntary program by which the payroll department deducts a pre-determined weekly, bi-weekly or monthly payment from your participating members’ paycheck. Each member has the option to enroll singularly or with family coverage. Participants are eligible to sign onto the program once every 12 months. There is a one-time initial enrollment fee of $20.00. When the member elects to extend the benefit for the following year(s), there will be no additional fee.

One Price Discount Plan
This non-insured, exclusive vision care program gives your members and their families an extremely affordable complete Eyecare and Eyewear Benefits Package that can be used on an UNLIMITED BASIS. One price covers Eye Exam, Frame and Lenses.
This one price discount plan is ideal for non-covered members and dependents or for those participants who have already used their benefits. The plan provides vision benefits not presently offered or to strengthen an already existing benefit package.
